Participant Assistance in Care

Participant Assistance and Care (PAC) service is a service provided on some Medicaid waivers.  The service will provide up to a day (24 hr. basis) of services and / or supports which are designed to ensure the health, safety and welfare of the individual, and assist in the acquisition, improvement, and retention of skills necessary to support the individual to live successfully in his/her own home. Services may be provided at the Agape Home or in an individual’s home within 20 miles of Berne.  Agape Respite Care encourages families to select their own in-home staff.  Training for the staff is provided by Agape Respite Care.

Eligibility

  • Approved for services on the Family Supports Waiver
  • Completed In-take meeting at Agape Respite Care
  • Accepted by Agape Respite Care for services
  • Completed Person Centered Plan and Individualized Support Plan with a qualified case manager and support team
  • Approved Notice of Action for services received by Agape Respite Care

Activities Allowed

Direct supervision, monitoring, training, education, demonstration or support to assist with

  • Behavioral Support Plan Implementation
  • Acquisition, improvement and retention of life skills
  • Assistance with meals
  • Shopping
  • Medical Appointments
  • Running Errands
  • Household care / chores
  • Leisure time activities and community exploration
